If the trip will be shouldered by a family member, a proof of relationship and general requirements, including employment certificate / business documents of the guarantor is required.
Additional documents may be required by the embassy without prior notice.
Personal appearance is not required (unless the embassy request for it)
Fill out and print in A4 size paper the visa application form, authorization, letter of guarantee (if applicable), and request form.
Submit the complete requirements to Travel Port and pay the visa processing fee.
We will lodge your application and pick up the result.
The result will be released after 5 to 7 working days (subject to change).

Duly accomplished and signed application form (must be typewritten and printed in A4 size paper)
1 pc passport size colored picture (white background)
Original passport (6 months valid)
Original Old passport with previous Japan Visa (if applicable)
Original bank certificate (issued within the last 3 mos upon application) with 6 months Average Daily Balance or ADB (If the bank certificate has no ADB, a 6 months bank statement is required)
Copy of latest income tax return (form 2316 with signature of employee and employer; for business ITR, please attach copy of receipt from BIR)
Original PSA authenticated birth certificate (issued within a year)
Original PSA marriage certificate (issued within a year – if married)
2 Original signed authorization letters (for visa filing and pick-up)
